Psychiatry Consultation Service Inpatient Rotation
32 hours per week for 6 months

Educational Activities:  Fellows receive 1:1 clinical supervision on cases combined with informal didactic and readings assignments related to patient care in real time.

Clinical Activities:  Fellows can be involved in as many as six consultations a day, but more typically are responsible for managing some consults independently and others in a supervisory role with other trainees. The complexity of the cases and the flow of the service dictate the pace with the goal of metering the workload to be educationally optimized with attendings managing any overflow.

Highlights:  The hospitalized veteran provides a very different educational opportunity for our fellows. In addition to being largely (but by no means exclusively) male, most of our patients are older and have substantial chronic medical burden. This requires fellows to expand their view of the patient well beyond the current hospitalization and to address the whole patient in a way that younger healthier patients do not demand. Faculty on this rotation have substantial ethics training, allowing them to help fellows parse between what is psychosomatic and what is ethics and sharpen their ability to liaise with hospital ethics committees. Also, the NMVMC rotation offers consult opportunities in a world-class inpatient spinal cord injury unit, inpatient hospice unit and in-house skilled nursing facility.
